David is an ATPL-rated pilot and Professional Engineer with vast experience as an Aviation Safety Investigator when with the Transportation Safety Board of Canada (TSB). He has more than 35 years of flight safety experience and has served as an investigator-in-charge, or as part of the investigation team, for a variety of major aviation occurrences in Canada and internationally. He was the Air Investigations team leader for the development and introduction of a new aviation safety investigation system database.
His aviation career commenced as a private pilot flying various light aircraft. He then became a military pilot in the Royal Canadian Air Force (RCAF) where he flew a variety of aircraft including the T-33, CF-5, C-130 Hercules and the Boeing 707-300. While in the Air Force he attended the year-long Canadian Forces Aerospace Systems course and then served as a systems flight test pilot. David has flown more than 9,000 hours and has current experience operating FMS-equipped aircraft.
David has provided training and lectures on the various facets of accident and incident investigation including human factors, identifying safety deficiencies, the formulation of safety action, witness interviewing, accident site procedures and other topics.
He was a Delegate for Canada at the 1999 and 2008 ICAO AIG Division Meetings that produced amendments to Annex 13. David was a member of the ICAO Working Group updating the Manual of Aircraft Accident and Incident Investigation Manual and participated in the work of the ICAO Safety Indicators Study Group (SISG).
